- A grant amendment form needs to be completed by PI (template can be found at https://www.nserc-crsng.gc.ca/_doc/Tri-Agency/TAFAG/Grant_Amendment_Form_e.pdf). Sections regarding institutional information will be completed by ORS once the form is received. The ORS will then verify and submit the request to CIHR.
